Abstract Objective To compare cytological and histological results from women > 64 years old who followed the Brazilian national cervical cancer screening guidelines with those who did not. Methods The present observational retrospective study analyzed 207 abnormal cervical smear results from women > 64 years old in a mid-sized city in Brazil over 14 years. All results were reported according to the Bethesda System. The women were divided into those who followed the screening guidelines and those who did not. Results Atypical squamous cells of undetermined significance and low-grade squamous intraepithelial lesion cytology results were found in 128 (62.2%) cases. Of these, 112 (87.5%) had repeated cytology with positive results. The other 79 (38.1%) with abnormal results should have been referred to colposcopy and biopsy. Out of 41 (51.9%) biopsied women, 23 (29.1%) had a confirmed diagnosis of neoplasia or precursor lesion. In contrast, among the 78 (37.7%) biopsied patients, 40 (51.3%) followed the guideline recommendations, with 9 (22.5%) positive biopsies. Of the 38 (48.7%) women who did not follow the guidelines, there were 24 (63.1%) positive results. Women who did not follow the guidelines demonstrated higher chances of cancer and precursor lesions (odds ratio [OR]: 5.904; 95% confidence interval [CI]: 2.188-15.932; p = 0.0002). Conclusion Women > 64 years old who did not follow the national screening protocol showed significant differences in the frequency of abnormal results and severity of diagnosis compared with those who followed the protocol.


Resumo Objetivo Comparar os resultados citológicos e histológicos de mulheres > 64 anos que seguiram as diretrizes nacionais brasileiras de rastreamento do câncer do colo do útero com aquelas que não as seguiram. Método O presente estudo observacional retrospectivo analisou 207 resultados anormais de esfregaço cervical de mulheres > 64 anos de idade em uma cidade de médio porte no Brasil durante 14 anos. Todos os resultados foram relatados de acordo com o Sistema Bethesda. As mulheres foram divididas entre as que seguiram as diretrizes de rastreamento e as que não o fizeram. Resultados Resultados citológicos com células escamosas atípicas de significado indeterminado e lesão intraepitelial escamosa de baixo grau foram encontrados em 128 (62,2%) casos. Destes, 112 (87,5%) repetiram a citologia com resultados positivos. Os outros 79 (38,1%) com resultados anormais deveriam ter sido encaminhados para colposcopia e biópsia. Das 41 (51,9%) mulheres biopsiadas, 23 (29,1%) tiveram diagnóstico confirmado de neoplasia ou lesão precursora. Em contrapartida, entre as 78 (37,7%) pacientes biopsiadas, 40 (51,3%) seguiram as recomendações da diretriz, com 9 (22,5%) biópsias positivas. Entre as 38 (48,7%) mulheres que não seguiram as orientações, houve 24 (63,1%) resultados positivos. As mulheres que não seguiram as diretrizes demonstraram maiores chances de câncer e lesões precursoras (odds ratio [OR]: 5,904; intervalo de confiança [IC] de 95%: 2,188-15,932; p = 0,0002). Conclusão Mulheres > 64 anos que não seguiram a diretriz nacional de rastreamento apresentaram diferenças significativas na frequência de resultados anormais e gravidade do diagnóstico em comparação com aquelas que seguiram a diretriz.

ABSTRACT

Introduction: Pulmonary carcinoma is the most prevalent cancer in the world, followed by breast cancer. It has high mortality rates in men and women mainly due to its ability to metastasize. Metastases from lung carcinoma to the breast are extremely rare. The first case described occurred in 1965 and since then there are few cases of this condition reported in the world medical literature. Case report: A 59-year-old woman who complained of low back pain in May 2017. The investigation revealed a metastatic site in the fifth vertebra of the lumbar spine with unknown origin. Six months later, a new lesion was found in the ninth vertebra of the thoracic spine. Immunohistochemistry showed positivity for cytokeratin 7 and TTF1 markers, confirming the hypothesis of lung carcinoma as the primary site. In March 2018, the patient evolved with a symptomatic nodule in the right breast on ultrasound and with positivity for TTF1 on immunohistochemical study, confirming the metastasis of lung carcinoma to the breast. Treated for two years until March 2020, when she presented multiple brain metastases. Patient had no therapeutic success and died. Conclusion: The difficulty in diagnosing lung carcinoma metastasis to the breast stands out, surgeons, clinicians and pathologists should consider this diagnosis, although rare.


Introdução: O carcinoma pulmonar é o câncer mais prevalente no mundo, seguido pelo de mama. Apresenta altas taxas de mortalidade em homens e mulheres, principalmente em virtude da sua capacidade de metastatizar. As metástases do carcinoma de pulmão para a mama são extremamente raras. O primeiro caso descrito ocorreu em 1965 e, desde então, há poucos casos dessa condição relatados na literatura médica mundial. Relato do caso: Mulher, 59 anos, apresentou dor lombar em maio de 2017. A investigação revelou um local metastático na quinta vértebra da coluna lombar sem descobrir sua origem. Seis meses depois, uma nova lesão foi encontrada na nona vértebra da coluna torácica. A imuno-histoquímica mostrou positividade para os marcadores citoqueratina 7 e TTF1, confirmando a hipótese de carcinoma de pulmão como sítio primário. Em março de 2018, a paciente evoluiu com um nódulo sintomático na mama direita ao ultrassom e com positividade para TTF1 no estudo imuno-histoquímico, confirmando tratar-se de metástase de carcinoma de pulmão para a mama. Foi tratada por dois anos até março de 2020, quando apresentou múltiplas metástases cerebrais. A paciente não teve sucesso terapêutico e faleceu. Conclusão: Ressalta-se a dificuldade no diagnóstico de metástase de carcinoma de pulmão para a mama e alertar clínicos, cirurgiões e patologistas para que considerem esse diagnóstico, embora raro.


Introducción: Carcinoma de pulmón es el cáncer más prevalente del mundo, seguido del cáncer de mama. Presenta elevadas tasas de mortalidad en hombres y mujeres, debido principalmente a su capacidad de metástasis. Las metástasis del carcinoma de pulmón hacia la mama son extremadamente raras. El primer caso descrito fue en 1965 y, desde entonces, existen pocos casos de esta afección en la literatura médica de todo el mundo. Informe del caso: Mujer, 59 años, comenzó a experimentar dolor lumbar en mayo de 2017. La investigación reveló un foco metastásico en la quinta vértebra de la columna lumbar sin descubrir su origen. Seis meses después, se encontró una nueva lesión en la novena vértebra de la columna torácica. Inmunohistoquímica mostró positividad para los marcadores citoqueratina 7 y TTF1, confirmando la hipótesis de carcinoma de pulmón como localización primaria. En marzo de 2018, la paciente evolucionó con un nódulo sintomático en la mama derecha en la ecografía y con positividad para TTF1 en el estudio inmunohistoquímico, confirmando que se trataba de una metástasis de carcinoma pulmonar a la mama. Tratada durante dos años hasta marzo de 2020, cuando presentó múltiples metástasis cerebrales. La paciente no tuvo éxito terapéutico y falleció. Conclusión: Destacan la dificultad en el diagnóstico de metástasis de carcinoma pulmonar a la mama y alertar clínicos, cirujanos y patólogos para que consideren este diagnóstico, aunque poco frecuente.

ABSTRACT

Heterotopic ossification has been described in papillary thyroid carcinoma in association with high incidence of extrathyroidal invasion, multifocality, lymph node metastasis, and older age. Nevertheless, it has not been described as a specific subtype of papillary thyroid carcinoma, because of its rarity. We described the case of an 80-year-old female patient, with familial history of papillary thyroid carcinoma. In the annual screening examination, she was diagnosed with thyroid nodules. The patient was submitted to a thyroidectomy because the fine needle aspiration cytology was positive for malignancy according to the Bethesda classification. The surgical specimen analyses showed a multifocal papillary carcinoma with one major lesion in the left lobe measuring 0.9 cm, and two small lesions (0.4 cm and 0.2 cm) in the right lobe. Only the biggest lesion in the right lobe had the osteoid matrix with rare osteoclasts and fat metaplasia with progenitor cells. There was perineural invasion, but vascular invasion was not identified. The margins were free and there was no extrathyroidal extension. In the left lobe there was an oncocytic nodule and a lipomatous follicular nodule. In recent years there has been a significant increase in the diagnosis of thyroid cancer, mainly because of the finding of microcarcinomas as a result of many requests for cervical image exams. Future studies may define (i) whether papillary thyroid carcinoma with heterotopic ossification is a true histological variant; (ii) the causes of that alteration; and (iii) eventual follow-up implications.

The present paper is a concise comparative review of the main primary cutaneous T-cell hematopoietic tumors, with emphasis on their immunohistochemical profiles.

Abstract: BACKGROUND: The incidence of melanoma has been increasing in Brazil and all over the world. Despite improvements in diagnosis and treatment, mortality remains unchanged. OBJECTIVE: To associate clinical and histopathological aspects with the evolution of 136 cases of cutaneous melanoma. METHODS: Retrospective cohort study that analyzed all patients diagnosed with melanoma during the period from 2003 to 2011, with at least 4 years follow up. Archived slides were analyzed to study histopathological variables (Breslow, ulceration, mitoses and histological regression). Medical records were used to retrieve clinical variables (age, sex, localization, time of appearance, diameter) and progression (metastases or death). Association measures were assessed by statistical analysis. RESULTS: There was no statistically significant difference between groups according to age. Superficial spreading subtype showed lower Breslow (0.5mm) than acral lentiginous and nodular subtypes (2 and 4.6mm respectively), less ulceration and metastases (9.4% against 50 and 70.6%). Nodular subtype had higher mitoses' median (5.0/mm2) than superficial spreading and lentigo maligna (0.0/mm2, for both). Regression was more frequent in superficial spreading and lentigo maligna subtypes. There were only deaths by melanoma in the acral group, however, there were deaths for other reasons in groups superficial spreading one, acral lentiginous one and lentigo maligna two. STUDY LIMITATIONS: Use of medical records as a source of data to the study. CONCLUSIONS: Superficial spreading subtype presents better prognosis indicators. Histological subtype should be considered in follow-up and treatment protocols of patients with cutaneous melanoma.

ABSTRACT

Abstract: The recently published 4th Edition of the World Health Organization Classification of Head and Neck Tumors addresses the most relevant and updated aspects of tumor biology, including clinical presentation, histopathology, immunohistochemistry, and prognosis of head and neck tumors. The objective of the present study is to compare these updates to the 3rd edition of that book with regard to mucosal melanomas and to highlight the potential factors that differ those tumors from cutaneous melanomas. We observed progress in the understanding of oral and sinonasal mucosal melanomas, which also present themselves, in the molecular scope, differently form cutaneous melanomas.

OBJETIVO: Comparar a frequência do resultado citopatológico de ASCUS em mulheres gestantes e não gestantes, estratificadas em grupos etários. MÉTODOS: Foram analisados 1.336.180 resultados de exames citopatológicos realizados de forma oportunística, no período entre 2000 e 2009 (10 anos) com a finalidade de rastreamento do carcinoma do colo do útero. Foram feitas comparações entre gestantes e não gestantes, com estratificação das amostras em três grupos etários (20-24, 25-29 e 30-34 anos). Foi utilizado o teste do χ2 e a medida da magnitude da associação foi analisada por valores estimados de Odds Ratio (OR) com intervalo de confiança de 95% (IC95%). RESULTADOS: Ao total, 447.489 amostras foram excluídas com base nos critérios adotados, totalizando uma amostra final de 37.137 mulheres gestantes e 851.554 não gestantes. O resultado citopatológico de ASCUS foi detectado em 1,2% dos casos, havendo diferença significante entre gestantes e não gestantes nas faixas etárias entre 20-24 anos (OR=0,85; IC95% 0,75-0,97) e 25-29 anos (OR=0,78; IC95% 0,63-0,96). Não houve diferença no grupo entre 30-34 anos (OR=0,76; IC95% 0,57-1,03). CONCLUSÕES: Este estudo sugeriu que mulheres não gestantes apresentam maior prevalência de ASCUS, mais evidente no grupo etário de 20 a 29 anos. A coleta do exame citopatológico não deve ser um exame compulsório na rotina do pré-natal. .


PURPOSE: To compare the frequency of an ASCUS Pap Smear result in pregnant and non-pregnant women, stratified by age group. METHODS: We analyzed the results of 1,336,180 cytopathologyc exams of Pap smears performed between 2000 and 2009 (ten years) with the purpose of screening for cervical carcinoma. Comparisons were made between pregnant and non-pregnant women, and the sample was stratified into three age groups (20-24, 25-29 and 30-34 years). The χ2 test was used and the magnitude of association was determined by the by Odds Ratio (OR) with the 95% confidence interval (95%CI). RESULTS: A Total of 447,489 samples were excluded on the basis of the criteria adopted, for a total final sample of 37,137 pregnant women and 851,554 non-pregnant women. An ASCUS result was detected in 1.2% of cases, with a significant difference between pregnant and non-pregnant women in the age groups of 20-24 years (OR=0.85; 95%CI 0.75-0.97) and 25-29 years (OR=0.78; 95%CI 0.63-0.96). There was no difference in the group between 30-34 years (OR=0.76; 95%CI 0.57-1.03). CONCLUSIONS: This study suggested that non-pregnant women have a higher frequency of ASCUS, most evident in the age group of 20 to 29 years. The collection of cervical cancer screening should not be a compulsory part of the prenatal routine. .

ABSTRACT

OBJECTIVE: To investigate mental health dropout rates in secondary care and to identify possible associations between this variable and social, demographic, psychopathologic, and health care process-related variables. METHOD: This prospective, observational study included 994 patients referred to a secondary service by four primary care units and evaluated by a specialist mental health team between 2004 and 2008. The dependent variable was treatment dropout. Bivariate analyses investigated possible associations between treatment dropout and 57 independent variables. RESULTS: The overall dropout rate from specialist mental health treatment was relatively low (mean = 25.6%). Only four independent variables were associated with dropout: one socioeconomic, two psychopathological, and one health care process variable. All associations were marginally significant (p < 0.1). CONCLUSION: Our findings suggest that family members, patients, and health care professionals are well engaged in this mental health care system based on a model of primary care. The use of this mental health model of care should be extended to other regions of our country (AU)


OBJETIVO: Investigar a taxa de abandono de tratamento no nível secundário de saúde mental e identificar possíveis associações entre abandono e variáveis sociais, demográficas, psicopatológicas e do processo de cuidado. MÉTODOS: Este estudo observacional, prospectivo, incluiu 994 pacientes referenciados a serviço secundário por quatro unidades básicas de saúde e avaliados por uma equipe especialista em saúde mental entre 2004 e 2008. A variável independente foi abandono de tratamento. Análises bivariadas investigaram possíveis associações entre abandono e 57 variáveis independentes de diferentes dimensões. RESULTADOS: A taxa geral de abandono de tratamento especialista foi relativamente baixa (média = 25,6%). Apenas quatro variáveis independentes se mostraram associadas a abandono: uma sociodemográfica, duas psicopatológicas, e uma relativa ao processo de cuidado. Todas as associações foram marginalmente significativas (p < 0,1). CONCLUSÕES: Os resultados sugerem bom engajamento de pacientes, familiares e profissionais nesse sistema de atenção à saúde mental focado no modelo da atenção primária. A utilização desse modelo de assistência em saúde mental deveria ser estendido a outras regiões do país (AU)

ABSTRACT

Poucos são os estudos que realizam comparações entre pacientes com transtornos mentais leves/moderados (TMLM) e graves (TMG), em especial na interface nível primário e secundário de atenção à saúde. Trata-se de um estudo observacional no qual se avaliou, prospectivamente, 876 pacientes encaminhados por cinco Unidades Básicas de Saúde de Juiz de Fora-MG que compareceram à primeira consulta no serviço especializado de referência em saúde mental da região oeste do referido município (CRRESAM--Oeste). Esses pacientes foram comparados quanto a variáveis demográficas; socioeconômicas; psicopatológicas e interativas; ligadas ao tratamento e aos processos de re-ferências e contra referências. Análises bivariadas foram realizadas para investigar possíveis associações (p-valores < 0,05) entre os TMG e 69 variáveis estudadas. Evidenciaram associações entre TMG e seis variáveis demográficas; três econômicas; 22 psicopatológicas e interativas; e cinco relativas ao tratamento e aos processos de referências e contra referências. As variáveis analisadas são oriundas dos prontuários clínicos semiestruturados que circulam no Sistema Municipal de Saúde de Juiz de Fora ou foram geradas através de combinações das variáveis ou foram coletadas a partir do formulário de referência ao nível secundário, preenchido pelos técnicos das UBS. Além de constituírem um parâmetro inicial para estudos relativos a comparações entre grupos de pacientes com TMG e TMLM, os resultados poderão auxiliar profissionais de saúde a melhorar a qualidade do trabalho de assistência à saúde mental.


Despite the large number of studies discussing the prevalence of psychiatry disorders in the general population, only a few compare patients with minor/moderate disorders with those with more severe ones, chiefly at the primary/secondary level interface. This article compares patients with minor/moderate psychiatric disorders to those with more severe ones, as refers to demographic, socioeconomic, psychopathological and treatment variables. This was an observational study, evaluating 876 patients, referred by five primary care units from Juiz de Fora- MG, Brazil, to a first consultation at the mental health secondary service that is a referral center for the western area of the city. Bivariate analyses were performed in order to establish possible associations (p < 0.05) between severe mental disorders and 69 variables included in the study. Of the variables, 6 demographic, 3 socioeconomic, 22-psycho--pathological and interactive, and 5 related to treatment or the referral/counter-referral process were associated with severe mental disorders. The variables analyzed came from the semi structured clinical files of the Juiz de Fora Municipal Health System, were generated from a combination of variables, or were collected from the referral form to the secondary level, filled out by the basic health unit staff. Besides being a baseline for studies comparing patients with severe mental disorders to those with less severe ones, the results may help to improve the quality of mental health care.
